Project Reporting Guidelines for
State and National Recognition and Awards

Please read this carefully!

Project Coordinators are asked to submit a simple report of their project to be counted among the hundreds of projects across the state AND to be eligible for state and national recognition!

* Optional Service Day!
Friday, October 26 is the MDDO Optional Service Day for those who cannot participate on Saturday due to their schedule or structure, including schools, afterschool programs, businesses, etc. This Friday option applies only to Ohio recognition and awards; it does not apply to national recognition and awards though USA WEEKEND magazine.

Eligibility

To be eligible for STATE recognition and awards, a significant part of your project must take place on Friday, October 26 or Saturday, October 27 (but, if for religious reasons you can’t participate, please participate on Sunday, October 28). Friday has been added for projects whose volunteers can only be together during the week. NOTE: Optional Service Day projects will only be accepted via reporting options #1 or #2 below.

To be eligible for NATIONAL recognition and awards, a significant part of your project must take place on Saturday, October 27 (again, if for religious reasons you can’t participate, please participate on Sunday, October 28).

How to Report
Please report via ONE of the following options by November 15, 2007.

OPTION #1 – MAIL Your Report to MDDO

MDDO REPORT FORM (Word)

Use the report form in the MDDO Resource Guide or print the one listed above (in box) and mail to: Ohio Community Service Council/MDDO, 51 N. High Street, Suite 800, Columbus, OH 43215.

(Hint: include action photos, media coverage and other materials that help tell your story. Materials cannot be returned and may be used in future promotions.)

OPTION #2 – E-MAIL Your Report to MDDO
Download the MDDO Report Form (Word), COMPLETE it, SAVE it, and ATTACH it to an e-mail to connie.kramer@ocsc.state.oh.us.

OPTION #3 – Report ONLINE to USA WEEKEND

REPORT ONLINE HERE


Click on Report Online to link to the national DAYtaBANK and submit your report directly to USA WEEKEND. If you have registered your project, you can return to your online registration and update it with your accomplishments (will need your username and password).

OPTION #4 – MAIL Your Report to USA WEEKEND
From the DAYtaBANK page, go to Project Tools>Entry Form which you can print, complete and mail.

All Saturday project reports submitted to the state MDDO office will be duplicated and forwarded to USA WEEKEND magazine for national consideration, and all online reports will be transmitted back to the state office. However, if you only submit a hard-copy report to USA WEEKEND, you are not guaranteed to be entered for a State Award.

The DEADLINE for reporting is Wednesday, November 15th. Thank you for helping keep Ohio #1!
